First I started off with two basic images:

I created the outline and mask of Bush's head.

Then I cut out a rectangle around the soccer ball and copied nearby grass over the rectangular area. I blurred the area to mostly get rid of the rectangular patterns from the copy and paste.

I then copied Bush's head onto the photo. Since he has a large head, I scaled it down. I also rotated the image to give it a less artificial feel (i.e. to make it look like it might be rolling).
