Please answer the following questions briefly for the assessment period indicated above.

  1. What graduate courses not including 590's did you take and what grades did you receive?
  2. What 590's did you take? For each 590 briefly explain what you did to participate.
  3. If you participated in independent study (600 or 800), who did you work with and for how many credits. For each independent study describe very briefly the nature of your project.
  4. If you were a TA or RA during the period, briefly describe your assignment.
  5. What were your sources of financial support this past year?
  6. Describe any milestones that you completed during the assessment period, if any. Milestones include Quals, General Exam/Dissertation Proposal.
  7. List any papers you wrote or published during this period. Describe where the paper was submitted or published.
  8. Describe any professional activities you participated in, such as refereeing, attending conferences, or giving external talks.
  9. Describe any service activities you participated in, such as serving on university, college, or department committees, serving as a grad student officer or committee chair/member, and/or volunteering for outreach activities (e.g., Engineering Open House).
  10. Describe any internship positions you held during the assessment period--what lab or company and your general job description. Was your work related to your research interests? Did it forward your own research agenda?
  11. In reviewing your goals for this past year, how did your accomplishments measure up to your planned goals?
  12. Please explain any circumstances which may have impeded your progress this year.
  13. Please describe the plans for next year. When, if you have not yet done so, do you plan to complete the Quals, General Exam, or Ph.D. Dissertation?
  14. What sources of financial support do you anticipate receiving for next academic year?
  15. Do you think you're making progress in the program? If not, please explain.
  16. Are you getting the advising you need? If not, what are you not getting.
  17. Is there anything you would like the Review of Progress Committee to take up on your behalf regarding the department or any person?
